We would be happy patient suggestions and criticisms of our practice. I'm not sure what the form of the word in a short paragraph. Any ideas would be appreciated.

You can use a general statement about the value of their business with you and wants to bring its contribution to improving the quality of care even further.
Then add a list of questions.
Do not take too long and there is no way to deny, too.
Some suggestions:
1. How long was the wait?
2. The reception was polite, and other employees?
3. Is attentive to the dentist? (Do not ask whether the filling or crown looked very good!)
4. If the officer / DA with you to review the instructions after the procedure?
5. Have answered your questions?
6. Respond to emergency situations, the dentist first?
7. How can we improve our service? (Never mind what went wrong! Do not want to go against everything you could in writing)
